What is Social Adult Day Care?
Social Adult Day Care (SADC) is more than just a place to spend the day. It's a structured program providing socialization, supervision, monitoring, personal care, and nutrition in a protective group setting. We also offer services such as transportation, information, and caregiver assistance. Our therapeutic activities aim to help participants achieve optimal physical and mental/cognitive functioning, improving their quality of life by reducing social isolation and increasing community engagement. SADC is an important part of community-based long-term care, delaying or preventing nursing home placement.

Who Can Enroll?
To enroll in a Managed Long-Term Care (MLTC) plan in New York State through ENERGIZE SOCIAL ADULT DAY CARE, an individual must: be 21 years of age or older; be eligible for New York State Medicaid; have a chronic illness or disability expected to last more than 120 days; require at least one or more long-term care services for more than 120 days (including nursing services, therapies, home health aide services, or Social Adult Day Care); and be able to live safely in the community at the time of enrollment.
